Additional InformationThis form asks for your permanent and mailing addresses, your email address, and your current phone number, among other things.
Application InformationThis form asks about the universities you have attended, your employment history, your test scores, and your list of professional publications.
Activities and Other Information
This form gives you the opportunity to tell the IU admissions staff what kinds of extracurricular activities you have participated in.
